ALL athletes, cheerleaders, and dance team members wishing to participate in any activity (including summer workouts) must have an updated physical and online forms completed before participation. Physicals must be dated after 4/27/2022 to be considered valid for the entirety of the 2022-2023 school year. 

Step 1: NCISD Online UIL Forms

Please complete online forms at the link above. The page takes about 30 seconds to load, please be patient. Once all information on all tabs have been completed, make sure to hit the "submit" button at the bottom of the page and make sure the submission screen loads before exiting. 

Step 2: UIL Physical Evaluation and Medical History Form

If you will be getting your physical done at your own doctor (you did not attend physical day at Porter High School), you will need to print this form to be completed by your doctor and turn it in to Ms. Young once it has been completed. If you complete the online forms first, you will receive a copy of this form to the email we have on file. 

 

Both of these forms must be completed before participation in any activity for the 2022-2023 school year (INCLUDING SUMMER SPEED & STRENGTH CAMP)

 

About Me:

This is my second year at Woodridge Forest Middle School. I grew up in Stephenville, Texas where I first discovered Athletic Training when I participated as a student athletic trainer from 8th grade until I graduated. I went on to get my Bachelor's degree from Texas A&M University in Kinesiology - Motor Behavior with minors in Spanish and Psychology. After that I went on to Tarleton State University where I graduated with 2 Masters degrees, 1 in Kinesiology - Health and Wellness and 1 in Athletic Training. 

 I am excited to be a part of New Caney ISD and can't wait to see what the student athletes are able to accomplish this year!
