Registration Check-List:

• Current Immunization Record
• Last Report Card and/or Official Transcript for grades 9-12
• Birth Certificate
• Confirmation of Acceptable Disciplinary Standing at Previous School
• Proof of Residency (Current Electric/Gas Bill or Receipt from Utility Deposit Reflecting the Name and Address of the Parent/Guardian; No other bills will be accepted)
• Proof of Guardianship
• Presence of the Parent/Guardian and the Enrolling Student
