Transition planning is a formal process required by the Individuals with Disabilities Education Act (IDEA) for assisting students receiving special education services in determining what they want to pursue after high school and what steps it takes to get there. IEP transition planning is more than just a hopeful exercise or brainstorming session. During the process, formal and informal assessments and inventories given to the student will provide a framework for developing targeted goals. The student will be supported by New Caney ISD staff and be provided specific services to assist in attaining the established post-secondary goals. Resources for transition are listed below.
